Health Care Provider Referral Requirement

A referral from your primary care provider or specialist is required for all patients. This referral is necessary to start your care with us and is different from an insurance authorization. The referral ensures that we have the necessary medical background to provide the most effective support, while authorizations (if required by your insurance) are specific to your coverage approval.

Medicare patients — Referral must be completed by a MD (Doctor of Medicine) or DO (Doctor of Osteopathic Medicine)
All other patients — Referral may be completed by an MD, DO, NP, or PA.

Our team of dietitians are in-network with Aetna, Anthem Blue Cross, BCBS, Blue Shield of CA, HealthNet, Medicare, United Healthcare, Brand New Day, Greater Covina Medical Group, Allied Pacific IPA, Advantage Health Network, Arroyo Vista Family Health Center, Greater Orange County Medical Group, Greater San Gabriel Physicians IPA and more.  Contact us for more information or if you don’t see your insurance listed. 

Insurance Details
Our goal is to make insurance navigation as simple as possible. Since coverage varies widely, here’s what you need to know based on your specific plan type:

Medicare

At this time, Medicare only covers nutrition therapy for those with diabetes, chronic kidney disease stage 3-5, and those that have gotten a transplant within the past 36 months. We are still able to provide services to those without these health conditions but you may be responsible for the cost of the visit. 

PPO Insurance Plans

Many PPO plans cover nutrition services, often with little or no out-of-pocket cost. However, coverage can vary based on individual plans. Using our Insurance Verification Form, you can confirm your specific coverage details before your appointment. Please reach out if you need help with this process

HMO and Managed Plans

For HMO/IPA plans, both a referral and prior authorization are typically required from your primary care provider to ensure coverage for nutrition services. This process allows your insurance to confirm eligibility and approve the service in advance. Please contact us for more information. 

Out Of Network

If we are out of network with your insurance, sessions may still be covered. Upon request, we can provide you with a superbill to submit yourself for possible reimbursement. Payment is due from you at the time of service in this instance.

For patients without insurance coverage or plans we are not in-network with, we offer a range of flexible self-pay options to make our services more accessible:

Standard Payment: Pay for individual sessions as you go, ideal for those who prefer flexibility or are scheduling on an as-needed basis. See our services page for pricing

Payment Plans: For clients seeking consistent care, we offer monthly or bi-monthly payment plans that allow you to spread the cost over time. This is a great option if you’re committing to a longer-term program or a series of appointments.

Discounted Packages: Save on per-session costs by purchasing a package of sessions. We offer bundles that are tailored to meet various needs and health goals, providing a discounted rate compared to single sessions. Packages may also include additional support options. Sliding Scale: For those who qualify, we have a sliding scale option to ensure our services are accessible to all, based on income and need. Please reach out for more information if this applies to you.
